Jawaharlal Institute Urban and Rural Health Centres, the field practice areas and training cum demonstration centres in Community Health of the Department of Preventive and Social Medicine were established as extension units of JIPMER.

Field service areas and infrastructure of the Health Centres

JIPMER Urban Health Centre:

The centre was started in 1964 as an Urban Health Training Centre of Jawaharlal Institute of Postgraduate Medical Education and Research (JIPMER). It is situated in Kurusukuppam, Puducherry, approximately 5 km from JIPMER. The JIUHC caters to the health care needs of around 10,000 population from four municipal wards, namely  Kurusukkuppam, Vaithikuppam, Vazhaikulam and Chinnaiyapuram.

The new three floor facility at Kurusukuppam was inaugurated in June 2021. The health centre is housed in a well-furnished building with an outpatient block, laboratory services, pharmacy, lift and ramp facility and other basic amenities. Antenatal clinic has provision of a fetal USG machine.  Fire safety system is installed in the building. The centre has an air-conditioned seminar room for teaching and training programmes and provisions for conducting health awareness campaigns.

For the year 2020 -21, the cumulative beneficiaries of our OPD services were around 25000. Monthly attendance at the OPDs is around 2100 and around 700 attend the chronic illness clinic regularly. Around 12000 lab investigations were performed in 2020-21.

JIPMER Rural Health Centre:

JIPMER Rural Health Centre is situated in Ramanathapuram village of Puducherry, approximately 13 km from JIPMER. The centre was started in 1966 as a Rural Field Practice Area of Jawaharlal Institute of Postgraduate Medical Education and Research (JIPMER). The centre provides health care services for a population of 10,461, of four villages namely, Ramanathapuram, Pillaiarkuppam, Thondamanatham and Thuthipet.

For the year 2020 -21, the cumulative beneficiaries of our OPD services were around 30524. On a monthly basis, an average of 2543 people benefit through it, including 700 that attend the chronic illness clinic. The emergency services average about 1050 per month and the total number of lab investigations conducted in 2020-21 were roughly 6000.

 Health Services provided at the health centres

Outpatient services: Outpatient services are provided in the centres from 9.00 AM to 4.30 PM on all working days by Medical Officers and Residents (MD trainee) and MBBS interns. Opportunistic screening of non-communicable diseases, relevant laboratory investigations and minor surgical procedures for wound care are carried out.

NCD Clinic overview  - This clinic deals with the management of non-communicable diseases like diabetes,  hypertension, bronchial asthma, hypothyroidism, epilepsy, cardiovascular diseases and mental  health disorders. Drugs are provided for a period of one-month, relevant investigations and early detection of complications are carried out. In addition to regular consultation, periodic visits by specialists from the Department of Psychiatry and Ophthalmology, JIPMER provide comprehensive care. On each clinic day health talks are delivered to the patients by Medical Social Worker / MBBS Interns encompassing various topics like life style modifications, need for adherence to medical management and self-care practices. To ensure uninterrupted treatment, the patients who fail to attend the clinics on scheduled days are contacted via phone or visited at home by the ASHA / ANM /MSW. The MSW motivates these patients to avail services and resume treatment.

Antenatal Clinic Overview - In this clinic, pregnant women are provided essential antenatal care, comprising of early registration, regular ANC check-up, provision of iron and folic acid (IFA) tablets,  immunization against tetanus, relevant laboratory investigations and planning for safe institutional delivery. Specialists from the Department of Obstetrics and Gynecology visit the clinic and provide specialist care for all antenatal mothers and high risk pregnancies The women are also counselled by the staff and Interns regarding nutrition, compliance to IFA tablets, personal hygiene, birth preparedness, infant care, importance of exclusive breast feeding and family welfare measures. High risk pregnancies which require further care are referred to JIPMER.

Under-five Clinic Overview - In this clinic services like growth monitoring, immunization and management of childhood illness as per IMNCI guidelines are provided to children below five years. Calendar for the dates of all immunization for children in the service area is maintained and if mothers fail to get the children on due date, they are contacted via phone or home visit to ensure timely vaccination. During the clinics, staff / MBBS Internship trainees deliver health talk to caregivers of the children on varied topics, such as Infant and Young Child Feeding (IYCF), hand hygiene, prevention and home-based management of diarrhoea, etc.

Implementation of National Health Programmes - All national health programmes are implemented in the service areas in coordination with the state health department in accordance with the national goals and strategies. Prominent among the programmes being implemented are National Health Mission (NHM), Reproductive Maternal RMNCH+A, National Tuberculosis Elimination Program (NTEP), National Programme for prevention & Control of Cancer, Diabetes, Cardiovascular Diseases & stroke (NPCDCS), etc.

 Field Activities 

• Home visit for ANC and PNC cases: ANC mothers are visited for regular checkup. Frequent visits for high-risk ANC mothers. Visits for Post-Natal Care are made as per the guidelines with advice given on breast feeding, nutrition of child and mother, vaccination, contraception and family welfare.
• House to house COVID Vaccination: Out reach COVID vaccination is given for bedridden patients and for those who hesitant to come to the center.
• VHND (Village Health & Nutrition Day): In one of the Anganwadis every month, nutritious food are distributed, health education given on nutrition, diabetic diet, hypertensive diet and under-five nutrition.
• School Health Programme: School immunization, IFA supplementation, deworming, height & weight measurement, health education regarding nutrition, menstrual hygiene, mental health and physical health is given.
• Vector surveillance: As part of Dengue Source Reduction, the potential sources for the breeding such as water holding pots, containers etc are identified and drained. Unused water sources if any are identified and treated with larvicides.
• Water surveillance: Water quality in service areas is ensured through vigilant monitoring done once in a month. Water samples from overhead tanks and taps in households is collected and sent for analysis to the Microbiology laboratory.
• Home visits to patients with Tuberculosis: TB patients are visited in their homes to ensure adherence to treatment and counsel the patients and their families about the diet, ventilation in the rooms, evaluation of health status and screening of family members to realise our vision of TB elimination and address social issues and support to TB affected families.
• Awareness campaign: Social Behaviour Change Communication (SBCC) is conducted on special days like IPPI campaign, Anti-malaria month, No tobacco day, Population Day, World health day, World TB Day etc.
• Verbal Autopsy: As per the guidelines given by the project Minerva, houses of deceased persons are visited and a standard verbal autopsy form is filled, along with a narrative in the local language Tamil.
• COVID related services: Vaccination for Health Care Workers and people above 18 years with COVISHIELD and adolescent 15-17 with COVAXIN is conducted. House to house fever surveillance and screening, home based care for isolated patients and door to door vaccination activities are carried out. Around 2000 samples have been sent for COVID – 19 testing.

Community based teaching & training

At the health centres teaching and training is provided for undergraduates and postgraduates in the areas of research methodology through community-based project work, Communication, IEC activities and health need assessment at community level, community participation and mobilization to improve healthcare service utilization.

In line with the mandate of Universal Health coverage, the training focuses on comprehensive care at primary care level including management of common ailments and provision of follow-up care for chronic illness patients and implementation of all National Health Programmes for population in the service area. Other areas of skill acquisition include outbreak investigation, review of registers and reports for various health conditions and understanding of Health Management Information System (HMIS) and reporting of health indicators to the State.
